The home, which is rated as excellent by the Care Quality Commission, has 45 residents who have a wide range of needs including complex and palliative care.
Since 2002 the highly trained complex care team have worked together to look after and improve the quality of life of people who would otherwise spend long periods of time in hospital.
The team work closely with the people they care for and their families to ensure that care is individually tailored to their needs and they have the best quality of life with choice and independence.
Home Manager Tracy Simpson said: “I’m thrilled that our nomination has gone through to the finals. The team work incredibly hard and always treat people with the dignity they deserve.
“The most important thing we do is to help people feel that they are valued and in control of their lives again. If they have acquired a disability they can lose a great deal of confidence and we help them to regain that and encourage them to fulfil their ambitions.”
Recently Tracy took Mark Boniface, a young man paralysed in a diving accident as he tried to save a swimmer, to meet Liverpool FC, his sporting heroes, at Anfield. Mark had been a life long fan of the team but had never seen them play. Tracy learnt of Mark’s ambition and wrote to the club who helped her make his dream come true.
It is this dedication to making a personal difference teamed with excellent clinical skills that have taken the team to the national award finals. Tracy said: “Nothing is too much trouble for our team and they are as supportive of each other as they are their clients, working closely together and sharing their skills and knowledge for the benefit of other people.
“The difference they make is hard to describe in words, the proof is in the laughter and joy in the faces of those people whose lives have been touched by them and that is why they deserve to win as a small way of saying a very big thank you.”
Tracy and her team beat off stiff competition from care homes across the country to take its place in the final, which takes place in London on Saturday 5th June.
